The Impact of Cholesterol Disorders and their Pharmacotherapy on Oral Health and Oral Healthcare: A Narrative Review

الملخص

فرط الكوليستيرول في الدم أو دسليبيدميا هو اضطراب أيضي شائع تقدر منظمة الصحة العالمية أنه يؤثر على 39٪ من السكان على مستوى العالم. يُعتقد أن بعض أمراض الفم، مثل التهاب اللثة أو التهاب دواعم الأسنان والحزاز المنبسط والتقرحات القلاعية المتكررة أكثر شيوعًا بين مرضى فرط كوليسترول الدم.  كما قد يؤثر سلبًا على زراعة الأسنان، ويعزز الإلتهاب حول الذروة الجذرية، ويزيد من تكلس حجرة اللب، وقد يكون مرتبطًا بسرطان الفم.

إن أدوية الستاتين المستخدمة في العلاج و خاصة أتورفاستاتين لها تأثيرات مضادة للميكروبات وقد تم اقتراحها كعامل واعد لتغطية اللب في علاج اللب الحيوي. كما أنها تعزز شفاء التهاب اللثة، والتئام الجروح، وتزيد من معدل نجاح زراعة الأسنان كما قد تساعد في علاج القرحات القلاعية البسيطة المتكررة والتهاب المفصل الفكي الصدغي.

تشمل التأثيرات الفموية الضارة للعلاج بالستاتين زيادة تكلس اللب وظهور شبيه الحزاز وجفاف الفم. إن قدرة سيمفاستاتين على تقليل هشاشة العظام، تجعل المرضى عرضة للإصابة بنخر العظم في الفكين المرتبط بالأدوية. يجب أن يكون ممارسي طب الأسنان العام والعاملون في مجال صحة الفم على دراية بالمظاهر الفموية لفرط كوليستيرول الدم و أدوية الستاتين من أجل تدابير علاجية أفضل وأكثر أمانًا لمرضى الأسنان.
